Medication Reviews
A medication review is a patient-care service to help patients fully understand their medication regimen and resolve drug therapy problems. Co-op Pharmacists provide a one-on-one, in-person appointment to identify all medications that the patient is taking, discuss what they are for and how they’re best taken and, where appropriate, create a plan to address any drug therapy problems. The pharmacist will check to ensure that each medication is appropriate, effective and safe and there are no adverse effects or interactions.
Medication Synchronization
Rather than making multiple trips to the pharmacy each month, medication synchronization means that patients can now have all prescriptions conveniently prepared for pickup on a single, coordinated day. Each month, the Co-op Pharmacy team connects with you in advance to confirm that there have been no changes to your medications and to notify you of the scheduled pickup date. Medications are prepared ahead of time, so they’re ready when you arrive, giving you peace of mind, saving you time, and opening up opportunities to have a conversation with your Co-op Pharmacist.
Compliance Packaging
Your Co-op Pharmacy team can help you manage your complicated medication schedule by organizing your medications in convenient personalized packaging. Compliance packaging can help you avoid missing doses of important medications and offer reassurance that you have taken your doses if you forget later. It addresses the challenge of medication non-adherence, which occurs when patients forget to take their medications, take the wrong dose, or discontinue treatment prematurely. Compliance packaging serves as a valuable resource in promoting safe and effective medication management, especially for individuals with chronic conditions or complex medication regimens.
Renew or Extend Prescriptions
To avoid an interruption in your treatment, your Co-op Pharmacist may be able to renew or extend a prescription when you have no refills remaining by working collaboratively with your prescriber. Extending your prescription does not replace the need to see your doctor for an appropriate medical follow-up and a new prescription. The purpose of the extension is to ensure that your treatment is not interrupted before your doctor’s appointment and that, in the meantime, you receive the medication you require on a regular basis and your treatment remains safe and effective.
Emergency Prescribing
In the absence of an existing prescription, but when there is an immediate need for drug therapy, a pharmacist may be able to provide an emergency prescription subject to provincial or territorial regulations.
